DESCRIPTION

Position is responsible for the coordination and implementation of all Clinical services offered to clients. Responsible for staff training and supervision of clinical issues as well as overall counselor supervision. Position is responsible for overall clinical case management of clients, to include medical, counseling, educational, vocational, and recreational services. This position will require on call services as needed. Duties include but are not limited to intensive in-home counseling, psychotherapy and group therapy for clients, assessment counseling, and advocating for the client, whenever necessary. Experience with family systems, juvenile courts, crisis intervention, and parenting skills helpful. Masters required. Must maintain a cell phone, home telephone and a DMV license. Position reports directly to Director of Services. Position requires successful completion of criminal history by the BIU of DBHDS and central registry background investigations by DSS.

Mental Health and Substance Abuse Counselor (QMHP)

DESCRIPTION

This position involves the provision of an array of counseling and vocational services within the home and community setting. Counselors are responsible for all related documentation and reports as well as the development and implementation of treatment goals and objectives. Facilitating clients support groups as needed is also expected. Work with individuals and groups to promote optimum mental health. May help individuals deal with addictions and substance abuse; family and parenting problems; suicide; stress management; problems with self-esteem; and issues associated with aging and mental and emotional health. This position reports directly to the Clinician. Counseling takes place during and afterschool hours. Position also requires successful completion of criminal history by the BIU of DBHDS and central registry background investigation by DSS.

TASKS:

  1. Participates in initial client assessment and ongoing reports following the approved format.
  2. Participates in the service plan.
  3. Have weekly contact with the Referring Agency.
  4. Arrange for weekly case supervision with clinical supervisor.
  5. Act as the liaison between family, referring agencies, and other community organizations.
  6. Act as co-leader of parenting and peer educator/support groups as needed.
  7. Coordinate client services between various agencies.
  8. Maintain case records and document services rendered.
  9. Attend weekly staff meetings consistently. Prior approval from management team is required when absent.
  10. Prior to the end of each thirty-day service period, coordinate and facilitate a staffing for the client,inviting the family of the client, the Referral Source, representation from other pertinent agencies providing
    services and supervision.
  11. Maintain high degree of client confidentiality.
  12. Work with other staff assigned to cases, communicating effectively.
  13. Maintain a high degree of professionalism in the community, with clients and with referring agencies seeking to
    build and maintain positive relationships.
  14. Assist in providing Individual and Family counseling services to clients in the home and community setting.
  15. Assist in providing Crisis Intervention as needed.
